New Patient Instructions
- Your Old Tests
If you have blood tests less than one year old, please bring them. If you will have blood work performed, blood-draws are easier if you drink plenty of water before coming in. Also, be sure you are warm.
- Your New Tests
We recommend a hair analysis for all new patients.
Please avoid using dandruff shampoos, hair dyes, perms, or other hair additives for as long as possible before your first visit. Avoiding these for one-two months will give us the most accurate hair analysis test. Regular shampoo is fine.
- At the Clinic
Due to patient allergies and chemical sensitivities, please refrain from wearing cologne or perfume.
- Confidential Patient Information Form
All new patients are require to write-in this confidential form before an exam or consultation.

- Payment
Payment is due at time of service. The first visit is typically one hour long. The hourly charge is $150.
We accept checks and cash. We do not accept credit cards.
We do not bill most insurance companies directly. We do provide you with a statement that you may copy and submit to your provider.
- Cancellation
If you need to cancel your appointment, please call us at least 24-48 hours before your appointment. We charge $50.00 for appointments not canceled at least 24 hours in advance. This allows us to keep the cost for our services at a minimum, and allows us to be available to patients who may need urgent care.
